Evidencing your disability or long-term health condition
To enable us to put support and/or adjustments in place for you, we ask you for information (evidence) that shows you have a disability or long term health condition. This helps us recommend the right support for you.
We find this information really helpful so we have given some examples of what that evidence might look like according to your condition below.

You can ask your doctor to fill out the relevant medical evidence form for your funding body. These can be found in the Disabled Student Allowance links by UK Country below.

How to share your supporting evidence
Once you have collected all your evidence please submit it by completing the Individual Learning Plan (ILP) application form. If you have already submitted an ILP form and are sending additional evidence, you can email it to: accessibility@fxplus.ac.uk.
After we have received your ILP request with your supporting evidence, an accessibility advisor will contact you so we can work together to consider your support requirements and any adjustments that may be suitable.
Examples of supporting evidence
For Specific Learning Differences (SpLDs) like dyslexia, dyspraxia, dyscalculia, dysgraphia and ADHD please provide:
- Any evidence you have of an existing diagnosis
- Evidence of previous support such as exam access arrangements
- Any reports from a medical or educational professional
Please note you may be advised that your existing evidence is insufficient to access the additional support you get via the Disabled Students’ Allowance (DSA), and you may need to have a full diagnostic assessment. You will need to submit evidence of an Autism diagnosis. See our Autism pages for more information about how to get a diagnosis if you do not yet have one.
For other disabilities, long term health conditions, or mental health conditions, you will need a letter from a doctor or another medical professional. Your evidence should include the following details:
- A diagnosis or working diagnosis of your disability/condition
- How long you have had your disability/condition
- The main symptoms of your disability/condition, and if relevant, any side effects from medication
- Details of study and examination adjustments you may require, for example a diagnostic report that recommends SpLD marking guidelines or a learning plan from school/college.
- Information on the accommodation adjustments you may require, for instance a letter from your GP that explains that due to a disability, an accessible bathroom is required.
