Supporting evidence for your disability or long-term health condition

To enable us to put support or adjustments in place for your disability or long-term health condition, we request that you share supporting evidence of your condition.

To help us in making support recommendations, it is helpful to share with us any evidence you have of your health and disability – some examples are given below according to your condition. If you are not sure what evidence to submit or would like to discuss it further, you can book an appointment with an advisor by contacting us at wellbeing@exeter.ac.uk or accessability@exeter.ac.uk.

How to share your supporting evidence

All evidence should be submitted through the Initial Individual Learning Plan (ILP) request form. If you have already submitted an ILP form and are sending additional evidence, you can email it to AccessAbility@exeter.ac.uk.

Once we have received your ILP request and supporting evidence, an advisor will contact you so we can work together to consider your support requirements and any adjustments that may be suitable.

You can take our Medical Evidence Form‌‌ to be filled out by a doctor or medical professional if you need medical evidence for conditions other than specific learning difficulties.

Specific learning differences and other neurodiverse conditions

Examples of specific learning differences include dyslexia, dyspraxia/developmental co-ordination disorder, dyscalculia and dysgraphia. Examples of other neurodiverse conditions include ADHD, ADD, Autism (ASC, ASD, Aspergers) and sensory processing disorder. However you may also have a specific learning difficulty or other neurodiverse condition that is not listed here.

For these conditions, please provide us with any evidence of an existing diagnosis, evidence of previous support such as exam access arrangements, or a report from a medical or educational professional.

Please note that to access the additional support you can get via the Disabled Students’ Allowance, you may be advised that your existing evidence is insufficient. And therefore, you need to complete a full diagnostic assessment. If you need to discuss how to get a full assessment, please book an appointment with an advisor who can suggest the best next steps.

Other disabilities and conditions

For other disabilities, long term health conditions, or mental health conditions, you will need a letter from a doctor or another medical professional. Alternatively, you can ask your doctor to fill out our Medical Evidence Form‌.

Your evidence should include the following details:

  • A diagnosis or working diagnosis of your disability/condition;
  • How long you have had your disability/condition;
  • The main symptoms of your disability/condition, and if relevant, the medication side effects;
  • Guidance on study and examination adjustments you may require;
  • Guidance on the accommodation support you may require.

Evidence in a language other than English

Please share with us any supporting evidence that you have, even if it is not in English. We may request a translation later. Please email us at with a copy of your evidence if you are unsure and would like to discuss whether you will need to provide a translation.
